For new and existing patients, access and fill out your medical and dental history forms prior to your appointments.

You are able to review your balance and make payments directly from your My Chart account.

If you have any difficulty, please call the office at (860) 667-0818.

You are welcome to arrive 15 minutes prior to your appointment and we are happy to assist you with setting up your My Chart account for our office.

Saturday 9am - 1pm (every other)
